Initial Intake Session

Service Description
This 45-minute virtual intake session is for families who have already decided to begin services. We’ll review your child’s history, strengths, and current needs, as well as your concerns and goals for therapy. This session helps us tailor services and ensure we’re starting with the right supports in place. ✅ Recommended before starting a new therapy plan ✅ Review IEPs, evaluations, and progress reports ✅ Get answers to questions and outline next steps Please complete the Parent Intake Form and upload any relevant documents prior to this session.
Cancellation Policy
Thank you for scheduling your Initial Intake Session with Bloom Therapy Resources! This 45-minute virtual session is required before starting ongoing therapy services. It helps us gather important information, review documents, and determine the best approach to support your child. 📅 Scheduling Appointments are available Monday–Friday between 8:00 AM and 12:00 PM (Mountain Time). Sessions must be scheduled at least 24 hours in advance. Please ensure all intake forms and supporting documents are submitted prior to the session to maximize our time together. 🔁 Rescheduling & Cancellations You may cancel or reschedule up to 24 hours before your scheduled session. Cancellations made with less than 24 hours’ notice may result in forfeiture of the session. If you are more than 10 minutes late without prior notice, the session may be marked as a no-show. 📥 Before Your Appointment To make the most of our time together, please complete the following: ✅ Submit the Parent Intake Form ✅ Upload your child’s IEP, evaluation report, or progress notes (if available) Upload & form links will be provided in your confirmation email.
